Several of our programs require applications, waivers, or other paperwork before you can enter the program. Please see the appropriate program below for the requirements:

Certified Nursing Assistant (CNA) Application

For this program, you will need to complete several application forms and a TB skin test. Once your forms are completed, please mail or fax all forms and TB skin test results to:

Medical Office Assistant (MOA) Application

Patient Care Technician (PCT)

For this program, you will need to complete several application forms and a TB skin test. You will also need to meed several prerequisite criteria. Once your forms are completed, please mail or fax all forms and TB skin test results to:

Prerequisites:
Applicants must be ONE of the following (and provide proof):

• Graduate of a Certified Nursing Assistant program
• EMT with at least one year’s experience
• First-year nursing student with at least 6 month’s clinical experience

Applicants must provide the following documentation:

• Current in CPR
• Proof of the following immunizations:
  • Negative TB skin test or chest X-ray results (no older than 6 months)
  • Hepatitis B series
  • MMR or Rubella titer
  • Tetanus (no older than 10 year
